Prospective Puppy Owner Questionnaire

 

PHILOSOPHY AND INTRODUCTION

 

Thank you for your interest in our Akitas. We carefully plan our breedings, which do not exceed twice a year, and usually are two-three years apart. Each prospective breeding dog and bitch have been carefully selected.

All Yutori Akitas used for breeding are OFA'd or preliminary x-rayed and certified by a Radiologist. We x-ray hips and elbows, and patellas are also checked. Eyes are CERF'D clear of PRA. We also perform a full thyroid test. All of the above tests are vital to assure that we produce healthy puppies. We do not believe in breeding female Akitas under the age of two years.

We belong to the Akita Club of America (ACA) and adhere to their Code of Ethics. To us at Yutori Akitas, a sound mind is as important as a sound dog. Our breeding dogs are also carefully screened for mental soundness. All of our puppies are raised in the home. They are socialized and part of our family. We plan to be a part of their life forever. We also consider our puppy buyers to be an extended family.

It is very important that we both agree about the welfare of this puppy as well as building a mutual respect and liking for each other. We would like for you, the prospective puppy owner to take a few minutes to answer our questions and to tell us a little about yourselves. This will help us to evaluate your qualifications to raise an Akita and to better match you with a puppy.

DISCLAIMER: This questionnaire is not an agreement to sell you a puppy. It also is not binding upon you. We ask these questions to determine your expectations, and whether we have a suitable puppy available. We reserve the right to refuse the sale of any puppy at any time during the interview or purchasing process. We require an interview and this questionnaire to be filled out and are very particular about good 'forever' homes for our puppies. Our contract is written to protect our puppies and dogs, as well as you, the buyer and we will enforce it.

Do you have any other dogs? Are they spayed or neutered? Tell us about them. If you do not own a dog now, but had one previously, tell us about that experience and what happened to the dog.

 

What other animals currently reside in your home and what kind? Are they spayed or neutered?

 

Do you intend to spay or neuter this puppy if you obtain one from us?

 

How long have you lived at your present address?

 

Do you live in an apartment or home with a yard? Please describe.

 

Are you willing to attend and complete an obedience class with your puppy? (Especially recommended if you have not had an Akita before).

 

Are you committed to caring for this Akita for its entire lifetime?

 

Where will the dog stay during the day while you are working? and when you are at home?

 

Are you familiar with the Akita Standard, and the breed as a whole? Describe your experiences.

 

Have you researched the Akita breed and do you understand the special responsibilities you have to responsibly raise and care for an Akita? Please explain.

After receiving this questionnaire, if we have puppies available or on the way, we will interview prospective buyers by phone as well as in person, whenever possible. We always prefer personal contact and find that meeting people face to face and having you and your family meet our dogs (before a litter is born if possible) is a good way to continue the process of determining whether an Akita is suited to your family. This is a good time for you to make those same determinations for yourself as well. This is a mutual inquiry intended to avoid poor placements and disappointments for the buyer, the breeder and most importantly for the puppy.

Deposit Notification:

IF we determine that you are a suitable home for one of our Akitas, we will require a deposit of $200 (cash, certified check or money order--no personal checks) from you to reserve a puppy. We ask that you give your preference of show or pet quality. If you have color preferences, we ask that you make a first and second color choice, and we will honor that if possible, depending on availability and your position on our list. However, color preference will not be a reason for a deposit return if we do not have your preferred color available. If we have a puppy that meets your needs regarding gender and show vs. pet quality, and coat length (normal vs. long) and you decide not to take the puppy, your deposit is forfeited. IF we don't have a puppy that meets your coat length criteria or show vs pet request, your deposit will be returned. If we accept a deposit and reserve a puppy, and you change your mind for any other reason, the deposit is forefeit. We reserve the right not to sell a puppy and to return a deposit if WE determine at any time prior to the actual sale, that a particular home is not in the best interests of the puppy.

Deposits are taken on a first come first served basis, with show quality puppy buyers having first choice of show quality puppies, and pet/companion buyers having first choice of pet/companion quality puppies.

All information is confidential, and will be used only as a screening tool to properly place our beloved puppies. We thank you for your assistance in filling out our form.
